An investigation into a rail accident in which a worker was struck by a train in Little Chalfont found that safety briefings she had received were “not effective”.
On Monday, May 15, the Rail Accident Investigation Branch (RAIB) released the report into the incident, which took place near Chalfont & Latimer Underground Station on April 15, 2022.
The woman, who was an agency worker, was acting as a second lookout on a planned patrol in a group of three when she was struck by a train travelling at 25mph.
She sustained head and body injuries and was taken to hospital. Though she was released the same day, she continues to suffer from the effects of the accident.
The RAIB report found that the worker was unfamiliar with the location where the accident took place, and that the day’s safety briefings were ineffective at helping her to work safely.
It also states that underlying factors included the fact that London Underground did not adequately control the risk to staff in high traffic hours, and that the company’s organisational culture did not support effective track safety management.
Four recommendations have been made to London Underground by RAIB.
